- Instructions: This form is for customers who purchase tickets and cannot give a physical carbon imprint, OR the tickets are for passengers different from the cardholder, even when you have an imprint. If you are concerned that this may be a fraudulent card, after completion and signed, please fax this form to 972 432 7636, or scan and email to [email protected], then contact Lawrence Lu at 972 503 7888 to validate the cardholder with the issuer bank.
- 3. Automated Clearing House (ACH) Authorization Form.
- The ACH Payment Service enables you to make electronic payments by directly debiting your checking or saving accounts.
- You, the customer, may make an authorization for either single time or recurring payments.
- You give authorization to debit directly from your checking or saving account for the purpose of bill payment. If you choose to do an ACH payment, you must first authorize USA Gateway Inc. to debit your bank account for the amount due. Authorization must conform to the requirements of the operating rules set up by ACH and must be either written and signed, or electronically displayed. See www.nacha.org for more information.

- When there has been any change in the details give to USA Gateway in the Agency Agreement, we require a new agreement to be completed and signed.
- Changes that require a new agreement include changes to your agency's owner, office name, office address, contact information, or other pertinent details.
